Incident/Accident Reporting Information




Supervisors must complete the:

OPP Incident Investigation Form

  • Complete page 1 of the form.

  • Inform your manager and Safety Coordinator within 24 hours of the incident.
  • Investigate incident with aid from the Safety Coordinator and complete page 2 of the form.
  • Submit completed form, including corrective actions to the safety office and your manager.

Note: In the event that an injury is deemed "recordable," a meeting shall be scheduled with the supervisor, manager and Safety Coordinator.
